Community Health Nurse Associate

East Lancashire Alliance in Lancashire is seeking a Nursing Associate to deliver evidence-based, patient-centred care within Maundy Relief’s community services. The post holder will act as a bridge between underserved communities and mainstream healthcare, under ANP supervision.

You will work as part of a multidisciplinary team to assess, plan, deliver and evaluate care, focusing on prevention, early identification of health needs and promoting health equity for Core20PLUS populations.
