Home-Based Community Health Assistant (Band 3)

Herefordshire and Worcestershire Health and Care NHS Trust is seeking a Band 3 Community Healthcare Assistant to join our Neighbourhood Teams in Worcestershire. You will deliver patient-centered home care, supporting independence and wellbeing under supervision of qualified nurses and therapists.

The post is a six-month fixed-term contract with potential to work across county locations. Shifts are rostered across 07:30-18:30, with flexible working options and development opportunities.
