We are pleased to offer this exciting opportunity to join the Intensive Older Peoples Service covering the Great Yarmouth and Waveney locality
We provide short term intensive community assessment and treatment for people with Dementia, as well as for those aged over 70 with a functional illness. Responding to emergency and urgent referrals, as well as those from our colleagues in the CMHTs.
We work closely with service users, families and care providers to agree a holistic and focused care and treatment package. We work as a multidisciplinary team to achieve the best outcomes for our service users. Our aim is to avoid hospital admission where possible but we will support this when necessary.
A full induction will be provided and although previous experience of working with older adults is an advantage, we welcome applications from those with varying backgrounds.
We offer a supportive and open team environment where you will be encouraged to share your ideas and be part of the development of the service.
We offer team supervision led by our psychology department as well as individual supervision and appraisal.
You will require excellent communication and organisational skills and the ability to work flexibly in a busy and supportive environment.
You will be responsible for the assessment, planning, review and delivery of care and treatment in conjunction with the service users and carers, both in their own home and care homes.
You will be responsible for assessing risk and developing management and safety plans in collaboration with the service users and carers
You will need the ability to plan safe discharge from the team as well as signposting and onward referrals as agreed with the service user.
You will be expected to complete physical health checks and alert any findings outside of normal range whilst incorporating health promotion principles into practice.
You will be responsible for clear and timely documentation
You will liaise closely with carers, families, care providers and the wider MDT to ensure that the appropriate care and treatment is provided.
You will have a good working knowledge of the Mental health and capacity acts.
Valid UK Driving licence is required for this post
