Advanced Clinical Practitioner – OOH

Overview

In this role you will provide advanced clinical care within our Out of Hours service. You will assess, diagnose, and manage patients presenting with undifferentiated or complex conditions, using both telephone and face-to-face consultations. You will work with autonomy to develop and implement management plans and ensure safe prescribing and follow-up. The opportunity combines clinical leadership with NHS pension and a supportive, mission-driven environment. This is a permanent role with flexible hours across multiple sites.

Responsibilities

• Assess, diagnose and manage patients with undifferentiated/complex illness
• Undertake telephone triage and face-to-face assessments
• Prescribe and review medications according to protocols
• Plan, implement and evaluate patient management plans
• Interpret pathology/radiology results and communicate findings
• Support patient adherence and provide information on medications

Key requirements

• Full NMC Registration with no restrictions
• Experience delivering autonomous care in urgent care, primary care, or out-of-hours settings
• Non-Medical Prescriber V300
• Clinical Examination and Diagnostics Level 7
• MSc in Advanced Clinical Practice is desirable
