Senior Frailty Specialist Nurse – Community Assessments

Provide CIC in Chelmsford is recruiting a Band 7 Nurse for the frailty virtual ward. You will hold a Non-Medical Prescriber qualification and an assessment and consultation course, with at least 3 years’ community experience, to assess patients at their home and support the team in preventing hospital admissions.

The post is full-time (37.5 hours), across 7 days with shifts 8am-4pm or 12pm-8pm, salary of £49,387-£56,515 per annum, and provides NHS terms.
