Gloucestershire County Council is seeking an Adult Social Care Practitioner to join our new Community Reablement Service. Based at Shire Hall, Gloucester, you will work in a countywide team to undertake Care Act assessments, develop person-centred support plans and maximise independence for adults.
You will be part of an integrated multidisciplinary team, applying Care Act, MCA and safeguarding principles, with hybrid working arrangements available.
