Job Description:
Oxleas NHS Foundation Trust is seeking a Continuing Care Assistant to provide direct clinical care to children aged 0-19 with complex health needs in homes and community settings. You will work within the Greenwich-based community children’s nursing team, delivering personalised care plans and supporting families in their own homes.
Responsibilities include following care plans, performing skilled clinical tasks, using home-care equipment, and coordinating with the Lead Continuing Care nurse.
