Home-Based Health Care Support Worker

Whittington Health NHS Trust is recruiting a Health Care Support Worker to join the Urgent Community Response Service. The post holder will work with therapy and nursing teams to deliver person-centred care in patients’ homes and care homes, reflecting NVQ3 competencies and helping prevent unnecessary hospital admissions.

The service runs from 0800-2200, 365 days a year. The postholder will be expected to work Monday – Friday 9am-5pm initially until probation is completed at which point changing
