Senior Practitioner, 16+ Care Leavers – Hybrid Role

Medway Council is seeking a Senior Practitioner to join the 16+ Care Leavers Service. You will support care leavers aged 16+ on their journey to independence and prepare them for transition to a personal advisor at 18.

You will lead the team, provide high‑quality case advice, contribute to training, and manage complex cases. A recognised social work qualification and ASYE are essential, with a driving licence and vehicle for work.
