Job Description:
Westminster City Council seeks a Social Worker for the Hospital Discharge Team based at St Mary’s Hospital. You will assess needs and develop support plans to enable safe discharge across Westminster and partner boroughs, working in a ward MDT setting.
You will undertake safeguarding investigations, Mental Capacity Act assessments, and work with carers and health colleagues to minimise risk while promoting independence. A CQSW/DSW or SW degree and registration are essential.
