PACT Social Care Practitioner — Build Trust & Independence

Wokingham Borough Council is recruiting two social care practitioners to join our Adult Social Care Team on a Full Time, Permanent basis. You will work within the newly formed PACT team to engage with adults who may not access traditional services, building trust and moving people toward safer, more independent living.

You’ll support assessments, manage risks related to hoarding and safeguarding, and collaborate with housing, health and community partners.
