In-Home Community Health Assistant – Patient Care & Monitoring

NHS Greater Glasgow and Clyde is seeking a compassionate Community Health Care Assistant to join the community nursing team. Based at Eastwood Health & Care Centre, you will deliver clinical care at patients’ homes and in community clinics, supporting independence and person-centred care across diverse settings.

The role requires a full UK driving licence and willingness to travel between community locations.
