Temporary Community Health Worker – Housing & Wellbeing

The YOU Trust is seeking a Community Health Worker to support individuals across Weymouth, Portland and the West of Dorset. The role involves 25 hours per week on a temporary 9‑month basis, delivering one‑to‑one support in clients’ homes and in the community to help maintain independence and wellbeing.

You will manage caseloads, assist with housing and benefits applications, budgeting and access to local services, while travelling across Dorset to reach clients. A driving license is required.
