Senior Hospital Social Worker: Discharge Lead (Locum)

LinsCare is recruiting an experienced Senior Practitioner to join the Hospital Team supporting Richmond and Wandsworth. This locum role focuses on hospital discharge planning and providing guidance to colleagues to support complex decisions and safe, timely discharge planning.

Qualified social worker registered with Social Work England is required, with hospital discharge experience and strong knowledge of the Care Act, Mental Capacity Act and safeguarding.
