Lead Social Prescriber – Team Leadership & Community Care

Tyntesfield Medical Group is seeking a Lead Social Prescriber to drive the social prescribing service, supervise staff and manage a patient caseload. The post requires collaborative work with health and social care professionals to connect patients with relevant supports and services.

You will undertake initial assessments, promote person-centred recovery plans and help overcome engagement barriers, while developing partnerships with VCSE and statutory providers to improve outcomes.
