Job Description:
Westminster City Council is seeking a Social Worker for the Hospital Discharge Team, based at St Mary’s Hospital and serving the Boroughs of Hammersmith & Fulham, Kensington & Chelsea, and Westminster.
You will undertake assessments of need, develop support plans for discharge, and conduct safeguarding and MCA assessments. You’ll work with carers and other professionals to minimise risk, joining the urgent response and duty system while keeping your professional registrations up to date.
