Lead Nurse, Dementia Rapid Response Team – Community

Derbyshire Healthcare NHS Foundation Trust is recruiting a Lead Nurse for the Dementia Rapid Response Team in Derby. The role delivers person-centred, non-pharmacological care to older adults with dementia in the community.

You will work with an MDT to prevent hospital admissions, travel across the catchment, and cover a 7-day service pattern with a 12-week roster, supported by supervision and training.

The post is permanent, full-time, with salary £39,959–£48,117 per year pro rata.
