Social Worker – Youth Transition & SAPT

Kirklees Council’s Specialist Adult Pathway Team (SAPT) is recruiting a Social Worker to join as Level 2, Level 3 or Senior Practitioner. The role focuses on helping young people transition to adulthood with coordinated support.

You will collaborate with families, Children with Disabilities, and partner agencies to plan smooth transitions as youths move into adult services, building meaningful relationships along the way.
