As an Adult Nurse Case Manager, you’ll take clinical responsibility for overseeing complex care packages, ensuring patients receive safe, personalised and high-quality care at home.
Your responsibilities will include:Managing a caseload of adult complex care packages across Bristol and North SomersetCompleting initial clinical assessments and ongoing patient reviewsOverseeing care for patients requiring tracheostomy care, invasive and non-invasive ventilationDeveloping and reviewing individualised care plans and clinical risk assessmentsProviding clinical supervision, guidance and support to care teamsDelivering clinical training and assessing staff competenciesConducting clinical audits and maintaining high standards of clinical governanceWorking closely with patients, families and multidisciplinary professionalsSupporting the recruitment, induction and development of care staffAbout YouNMC Registered Adult Nurse (RGN)Experience in Intensive Care (ITU/ICU), Critical Care or community complex careStrong clinical knowledge of tracheostomy care and ventilationConfident assessing and managing patients with complex clinical needsExcellent communication, clinical assessment and organisational skillsAbility to provide clinical guidance and support to care teamsFull UK driving licence and willingness to travel across Bristol and North SomersetPrevious Nurse Case Manager experience isn’t essential. We’d particularly welcome applications from experienced ITU/ICU Nurses looking to move into community care, as well as established Complex Care Nurse Case Managers seeking a new opportunity.
