Overview
In this role you deliver autonomous, expert clinical care in community hospital services to support patients with complex and frailty-related needs. You collaborate across organisational boundaries to improve care quality, admission avoidance and timely discharge. You will lead audits and promote evidence-based improvements, contributing to a growing Advanced Clinical Practice program. Responsibilities
- Deliver autonomous advanced clinical assessment, diagnosis and treatment
- Manage patients with complex, frailty-related and deteriorating conditions
- Coordinate patient care across organisational and professional boundaries
- Support admission avoidance and timely discharge planning
- Lead and contribute to clinical audits and service improvements
- Promote evidence-based practice and continuous quality improvement
Key requirements
- Registered healthcare professional
- MSc Advanced Clinical Practice
- Independent Prescriber qualification
- Significant post-registration clinical experience
- Current full driving license and use of a car during working hours
