Social Worker: Transition Team (16+), Make a Difference

Wakefield Council is recruiting a post qualifying social worker for our Transition Team in the Wakefield district. You will work with partners in children’s teams and CTLD health to carry out person-led assessments for those aged 16 and over transitioning into adult social care. The team consists of 5.5 social workers.
