Health & Wellbeing Lead for Integrated Care

Erewash Health Partnership is recruiting a Health and Wellbeing Practitioner to provide clinical and operational leadership for the WE: Flourish team across PCN sites. You will oversee referral flow, triage, quality conversations and MDT coordination to deliver person-centred support for adults with complex needs.

The role requires a registered clinician with leadership experience, strong collaboration skills and a commitment to reducing health inequalities within the PCN network.
