Hospital Social Worker – Discharge & D2A Specialist (Hybrid)

Tripod Partners is seeking an experienced Adult Social Worker to join the Hospital Social Work Team at University Hospitals Dorset. The role focuses on coordinating safe, timely discharges using Discharge to Assess pathways and working with MDTs to support adults with complex needs.

The successful candidate will have current Social Work England registration and hospital-based social work experience, with strong communication and assessment skills in a fast-paced setting.
