Health and Beyond is seeking a Diabetes Specialist Nurse to deliver high-quality diabetes care across multiple sites in Birmingham and Wolverhampton. The role involves autonomous patient assessment, education, and running clinic sessions, with close collaboration with the diabetes team to improve health outcomes.
The post holder will manage diabetes care plans, review medications, and support care transitions for patients across community and home settings within a 37.5-hour week.
