Independence & Wellbeing Practitioner (Hybrid)

Suffolk County Council in Felixstowe is seeking an Independence and Wellbeing Practitioner to join the Felixstowe Integrated Neighbourhood Team on a hybrid basis. This permanent, part-time role (24 hours per week) focuses on helping adults live independently and achieve meaningful outcomes.

You will assess needs, provide guidance on equipment and community supports, and work with health, social care and voluntary partners to create personalised plans.
