Occupational Therapist – Independent Living & Reablement

Oxfordshire County Council’s Home First Team is seeking an Occupational Therapist to support people leaving hospital, promoting independence and safety at home through assessments, reablement, equipment and adaptations.

You will work with social workers, discharge colleagues, families and carers, balancing risk with independence and contributing to person-centred care plans for safe, timely discharge and ongoing community living.
