Impactful OT for Hospital Discharge

Lancashire County Council is seeking an Occupational Therapist to support safe, timely and person-centred hospital discharges. You will work closely with health colleagues, families and partners to move people from hospital to the most appropriate setting, promoting recovery and independence.
