Adult Social Worker needed for our Prevention & Hospital Discharge Team in Lancashire, paying £35 ph!

About the role:

We are currently seeking an experienced Adult Social Care professional to join the Prevention and Hospital Discharge team in Thornton-Cleveleys, Lancashire.

This is an excellent opportunity for a motivated practitioner with strong assessment skills, experience working in a fast-paced environment, and a sound understanding of adult social care procedures. The successful candidate will play a key role in supporting timely hospital discharges and ensuring adults receive the appropriate care and support to meet their needs.

Key Responsibilities:

• Complete comprehensive Care Act assessments to identify individuals’ care and support needs.
• Support effective and timely hospital discharges, promoting independence and positive outcomes.
• Work at pace in a busy environment, managing competing priorities and caseloads.
• Undertake and contribute to Decision Support Tool (DST) assessments.
• Identify appropriate care packages, support plans, and community-based services.
• Work collaboratively with multidisciplinary teams, health professionals, and other partner agencies.
• Promote a person-centred approach, ensuring individuals’ needs, preferences, and wellbeing remain central to decision-making.

Requirements:

• Experience within Adult Social Care, ideally in prevention, hospital discharge, or intermediate care services.
• Proven experience completing Care Act assessments.
• Experience with Decision Support Tools (DSTs).
• Ability to work effectively under pressure and manage a fast-paced workload.
• Strong assessment, communication, and organisational skills.
• Knowledge of relevant adult social care legislation, safeguarding, and person-centred practice.
