Child & Family Support Specialist

Service Care is seeking a dedicated Community Care Worker for the Children’s Services team. You will support social workers, assess needs, coordinate services and contribute to care planning to improve outcomes for vulnerable children and families.

You will work under guidance, manage a caseload of less complex cases, ensure care plans are implemented, and liaise with partners across education, health and voluntary sectors to deliver person-centred support.
