Reablement Support Worker (Home‑Based)

Cornwall Partnership NHS Foundation Trust is seeking a part-time Community Support Worker to join the Adult Community Care team. The role focuses on delivering reablement and rehabilitation in patients’ homes across Penwith and surrounding areas, including weekends and holidays as required.

You will work with a multidisciplinary team, supporting older adults to maintain independence, with bespoke training provided. Travel across Cornwall is a key requirement of the role.
