THE ROLE

• Assess, diagnose and manage patients with acute and chronic conditions
• Prescribe independently within your scope of practice
• Deliver high-quality, patient-centred care
• Work closely with GPs, nurses and the wider multidisciplinary team
• Help develop and improve services across both practices

ABOUT YOU

• Qualified Advanced Nurse Practitioner / Advanced Clinical Practitioner
• Experience within primary care
• Excellent clinical assessment and communication skills
• Passionate about delivering outstanding patient care
• Friendly, approachable and a strong team player
