Mental Health Hospital Discharge Team Lead

Liverpool City Council is seeking an experienced Team Manager to lead the Mental Health Hospital Discharge Team within Adult Social Care and Health. You will oversee complex discharge planning, ensure legal compliance, and work with health partners to move individuals to appropriate community settings.

You will provide operational leadership, supervise staff, monitor performance and drive improvement in discharge pathways while promoting person-centred, trauma-informed practice.
