Mental Health Social Care Facilitator — Hybrid Role

Walsall Council is seeking a Social Care Facilitator in the Mental Health Team to carry out Care Act assessments, support planning and reviews for adults with mental health needs. You will work with individuals, families and partner agencies to promote wellbeing and independence, using a strengths-based approach to identify outcomes that matter to each person.
