Experienced RMN – Dementia Care & Community Health Trainer

Nottingham CityCare Partnership CIC is recruiting an RMN to work in the community, collaborating with district nurses, rehabilitation staff and mental health professionals. You will case manage residents with dementia in care homes and contribute to staff training and escalation paths within NHS-funded care frameworks.

The post involves autonomous clinical practice, multidisciplinary planning and ongoing development of high standards of practice.
