Hybrid Community Social Worker – Discharge to Assess

Manchester City Council is seeking a Hospital Discharge to Assess community social worker for 35 hours per week in a hybrid role. You will assess, plan and coordinate services to support safe discharges and home first principles, liaising with health colleagues and crisis response teams.

The role requires Social Work qualification, Social Work England registration, DBS clearance, and an Essential Car User. The post is permanent and sponsorship is available.
