Prison-to-Community Health Reconnect Practitioner

Reconnect Practitioner role is based in East Lancashire, covering 6 prisons & the wider community, helping people transition from prison to community-based healthcare and support services.

You will screen and assess individuals approaching release for vulnerabilities, coordinate referrals, and provide post-release communities-based support in partnership with health and social care teams. LS&C NHS Foundation Trust supports flexible working and diverse staffing.
