Community Nurse – Wound Care & Home Health Visits

Mid Cheshire Hospitals NHS Foundation Trust is recruiting a Band 5 Registered Nurse to join the Nantwich Community Team in a 37.5 hour per week, 7 day service. The role requires effective communication, reliability, and adaptability to provide high-quality holistic care in the community and wound care clinics.

You will assess care needs, implement and evaluate care plans, support peers and team leadership, and liaise with other healthcare professionals to ensure excellent patient outcomes.
