North Uttlesford ICCT Team Lead — Community Health Leader

Essex Partnership University NHS Foundation Trust seeks a Team Lead for the North Uttlesford Integrated Community Care Team. You will lead the Uttlesford North Integrated team, oversee clinical care and supervision, and work with the Community Nursing team to deliver high-quality services in the community.

You will coordinate with Social Services, Western area teams, and the CCC to keep care in the right place at the right time, supporting patients and families across Saffron Walden.
