Prison Medicines Optimisation Clinical Pharmacist

Oxleas NHS Foundation Trust is recruiting a Pharmacist to provide medicines optimisation services across Sheppey Prisons, including HMP Elmley. The role includes conducting medicine reviews, reducing risk with high‑risk medicines, and developing prison formulary in liaison with the prison team.

You will lead education for the primary care team, support GMP processes, and work within our health and wellbeing model to improve prescribing practices across the prison healthcare setting.
