District Nurse Lead — Community Home Care

Salford Royal NHS Foundation Trust is seeking a District Nurse/Charge Nurse to join the Community Nursing Team in Rochdale. You will deliver high quality, patient-centred care in patients’ homes and lead a diverse caseload with autonomy and clinical leadership.

The role requires strong assessment, care planning, and coordination with multidisciplinary teams to support independence and wellbeing; you will mentor colleagues and drive service improvements within a 7 days a week operation.
