You will assess individuals and families to maximise independence at home and in the community, working within a multi-disciplinary team, with a focus on effective partnership working and ongoing professional development.

The role involves carrying out proportionate assessments and managing own workload in a rolling monthly contract, with negotiable rate of pay.
