Community Mental Health Nurse: Collaborative Care Leader

NHS Jobs in Rochdale is seeking a dedicated Band 5 practitioner to join a CMHT-based team. You will act as a key worker, manage a caseload, and deliver evidence-based interventions, assessing risk and formulating care plans with the MDT and carers.

You will monitor progress, maintain electronic records, and participate in depot injections when clinically appropriate. You will communicate with service users and families, supervise Band 5s and support workers, and contribute to the duty rota.
