Care Home Rehabilitation Assistant – MDT Support

Epsom and St Helier University Hospitals NHS Trust is seeking a Rehabilitation Assistant to join the Care Home Support Team. The role provides proactive, person-centred rehabilitation and support to residents in care homes, particularly older people living with frailty and complex health needs.

Working under supervision of registered clinicians, you will deliver delegated rehabilitation programmes, support continence care, and help embed rehabilitation into everyday care.
