Community Inclusion & Digital Outreach Officer

Cardiff Council is seeking a motivated individual to support older people and adults with physical impairments who are socially isolated. The role focuses on motivation interviewing, helping clients connect with community activities, and addressing transport or digital inclusion barriers.

You will manage a personal caseload, assess needs, research suitable venues, and help clients achieve their agreed outcomes while ensuring their safety and dignity.
