Lincolnshire Community Health Services NHS Trust is recruiting an Assistant Practitioner to join Adult Community Therapy across the South East locality (Spalding, Boston, Bourne and surrounding areas).
You will carry out holistic assessments in patients’ homes or care homes, coordinate care with registered therapists and therapy assistants, and help meet rehabilitation goals to maximise independence. Travel around the county is required.
