Community Health Care Assistant – Home Visits

Central London Community Healthcare NHS Trust is recruiting a Band 3 Health Care Support Worker to join our community team in London. You will deliver high-quality care at patients’ homes, supporting independence and achieving individual goals under supervision of a Registered Nurse or Allied Health Professional.

You will work across the community, including lone working, with access to on-call support as required.
