Integrated Care GP for Housebound & Care-Home Patients

Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust is seeking a General Practitioner to provide comprehensive, person-centred care to a defined cohort of housebound and care-home patients, with a weighted list of about 130 patients per session.
