Lead District Nurse – Community Home Care & Coordination

Northern Care Alliance NHS Foundation Trust in Rochdale North is recruiting a District Nurse/Charge Nurse to join our Community Nursing Team.

You will deliver high-quality, patient-centred care to people in their own homes, assess and coordinate care, and work with a multidisciplinary team to enable independence.

This role supports a seven-days-a-week service across Greater Manchester, with opportunities for leadership development and impact on patient outcomes.
