Clinical Nurse Specialist – Hospice Outreach (Community Care)

Hospice in the Weald is seeking a clinically skilled nurse/paramedic to join the Hospice Outreach Service. You will deliver high-quality palliative care in patients’ homes, assess needs, and collaborate with primary care teams to optimise symptom management and goals of care.

You will provide expert advice, support families, and participate in MDT meetings to ensure care aligns with patient preferences and safety standards.
