24/7 Crisis Care Nurse – Multidisciplinary Mental Health Support

East London NHS Foundation Trust is seeking a Crisis Practitioner – Nurse to join the Crisis Resolution Home Treatment Team. The role involves working within a multidisciplinary service to assess referrals, conduct home treatment, and deliver risk-based care to individuals in mental health crisis.

You will contribute to gatekeeping, collaborate with GPs, CMHTs and acute wards, and promote recovery through psycho-social interventions and early discharge planning.
